The AAIS Homeowner program rests upon the foundation of
several base policy forms. They handle a variety of residential property
ownership situations including apartments, condominiums, townhomes, some mobile
home situations, and single-family housing. Their program is also flexible
enough to handle such residences that contain minor farming exposures.
Use of base forms facilitates program/coverage updates and
even makes a total program revision easier to implement and less disruptive
since different companies may choose to remain loyal to different program
editions. Use of base policies also reduces the chance that changes to a
particular area will affect the entire Homeowner program.
The AAIS optional endorsements are valuable for addressing certain
exposures unmet by a base homeowner contract. As these endorsements provide
additional coverage, they typically require additional premium.
The AAIS form’s numbering consists of two letters (primarily
HO for this line of business), followed by a four-digit form number, then the
form's edition date, expressed as mm yy.

HO 1300–Asbestos
Exclusion
This endorsement prohibits coverage for bodily injury or
property damage liability claims that are connected to exposure to any form of
asbestos. The bar against coverage also extends to asbestos-related personal
injury claims IF the underlying policy otherwise responds to personal injury
losses.

HO 1323–Liability
Exclusions - Lead
This endorsement is an additional exclusion that applies to
Coverage L, Personal Liability and Coverage M, Medical Payments to Others. It
excludes coverage for any bodily injury, or property damage due to exposure to
lead. The exclusion extends to any costs or expenses involved in treating, testing,
or monitoring property for lead exposure. Further, coverage is barred for any
claim or suit filed by any government authority that is related to damages
created by an exposure to lead.
HO 1335–Liability
Exclusions - Pollutants, Bacteria, Fungi, Wet Rot, and Dry Rot
This endorsement modifies the liability portion of the
Homeowner program to exclude coverage for any bodily injury or property damage
that is related to any loss caused by pollutants (hostile fire pollution is excepted). The exclusion extends to any costs or expenses
involved in treating, testing, or monitoring property for exposure to any type
of pollution. Further, coverage is barred for any claim or suit filed by any
government authority that’s related either to pollution damages or to efforts
to assess, monitor, or correct any exposure to pollutants. The pollution
exclusion applies under both Coverages L and M.
Finally, also under Coverage L, coverage is barred for any
claim or suit filed by any government authority that’s related either to
bacteria (fungi and rot) damages or to efforts to assess, monitor or correct
any exposure to bacteria (fungi and rot).
HO 1347–Punitive or
Exemplary Damages Exclusion
This endorsement modifies the liability portion of the
Homeowner program to exclude payment for punitive or exemplary awards,
including any related costs or interest. Defense of such claims is provided
only if the claim is part of a compensatory suit.
HO 2001–Exclusion - Injury
Arising Out of a Canine
This form allows an insurer to write an HO policy that
includes what it deems an unacceptable liability exposure to a dog-related
loss. The form must be signed by the named insured and it requires a
description of the excluded animal. The form applies to the described animal,
whether owned or in the custody of the insured household.
HO 2002–Cosmetic
Damage Exclusion – Exterior Wall Surfacing, Roof Surfacing, and/or Exterior Door
and Window Surfacing
(Used with all basic HO forms except HO 0008)
This provides the
option to customize building protection. Specifically, it can be used to
exclude protection for cosmetic (minor) damage caused by wind or hail to the
exterior surfaces of walls, roofs and/or doors/windows. Any, all, or a
combination of the three options may be selected.
The form adds the defined terms of “cosmetic damage,”
“exterior wall surfacing,” “roof surfacing,” “exterior door surfacing” and
“exterior window surfacing.”
HO 2003–Exclusion –
You Live at a Location Other Than the Described Location
This form terminates coverage for a named insured once that named
insured has lived at least 61 days at a location that is different than the
policy’s described location. The exclusion does not apply to temporary
situations. However, “temporary” is not defined.
HO 2089–Limited
Bacteria, Fungi, Wet Rot or Dry Rot Coverage
This is an option that creates a modest amount of protection
against mold and rot damage. It adds incidental property coverage, as well as a
limited amount of insurance under Coverage L. The incidental coverage works by
deleting the applicability of the basic (unendorsed) policy's exclusion against
fungi, rot, and bacteria loss. An insured may select a limit for both the
incidental property and the Coverage L extension. The liability amount is a
sub-limit and is also an annual aggregate.
HO 2530–Incidental
Property Coverages - Higher Limits
This form may be purchased in order to increase the coverage
provided by the basic policy’s Incidental Property Coverages. Limits may be
increased for Fire Department Service Charge, Credit Card, Electronic Fund
Transfer Card or Access Device, Forgery and Counterfeit Money, Refrigerated
Property, and Tenants’ Improvements and Association Deductible.
HO 2542–Theft
Coverage Increase
(Use with HO 0008)
This is an option that adds additional theft protection,
(both on- and off-premises) to the severely modest amount of coverage provided
by the base (unendorsed) HO 0008, Limited Form. The form adds theft coverage in the amount shown in the form’s schedule. It also adds
theft protection equal to the Coverage C Property Limitations (for theft) that is
found in the HO Program’s other base forms.
HO 2550–Loss
Assessment Coverage
(Use with all forms except HO 0008)
The basic amount ($1,500) of coverage for loss assessments
may be increased by this form. The form must list the additional amount and
total limit of loss assessment coverage. However, coverage is limited to $1,500
for assessments that are made due to the insured’s association’s insurance
deductible.
HO 2557–Increased
Cost - Ordinance or Law - Increased Limit of Coverage
(Use with all forms except HO 0008)
This endorsement replaces the basic property policy’s
incidental coverage for Increased Cost—Ordinance or Law. Instead of limiting
coverage for cost increases due to a law or ordinance to 10% of the Coverage A
limit, the insured may select a percentage to cover this source of increased
expense.
HO 2565–Coverage C - Higher
Limits on Certain Property
(Use with all forms except HO 0008)
This endorsement does not increase the limit of insurance
shown for Coverage C—Personal Property; rather, it increases the sub-limits
provided under the Coverage C Limitations on Certain Property coverage.
HO 2581–Property in
Rental units - Increased Limits
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
This form permits one or more rental locations to be
described on the form along with additional, separate limits of coverage
selected (and charged) for an insured’s personal property contained in those
locations. The limits appearing on the form represent amounts that apply
separately to the applicable property and do not, otherwise, affect the base
form’s Coverage C Limit.
HO 2584–Automatic
Adjustment of Limits
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
Allows an insured to select an annual percent increase to be
applied to the limits of insurance for Coverages A, B, C and D. Note that the
annual impact is accurate because the increase is pro-rated over the policy
period.
HO 2701–Association
Deductible Coverage - Earthquake
(Use with all forms except HO 0008)
This form adds Incidental Property Coverage for association
deductibles that are due to earthquake. The limit is per location but is
subject to the amount actually assessed against the insured by the association
to pay for the earthquake deductible on the association’s earthquake policy.
The insured must pay a percentage deductible as indicated on the schedule but
no less than $250.
HO 2702–Equipment
Breakdown Coverage
Coverage is provided for mechanical or electrical breakdown
of property covered under Coverages A and B that generates transits or utilizes
energy up to the limit shown in the schedule. Covered equipment does not
include kitchen and laundry equipment or electronic computer or electronic
equipment.
Coverage can include additional living costs and loss of
rent and refrigerated property if a limit is indicated in the schedule for the
desired coverage.
HO 2703–Equipment
Breakdown Coverage - Include Permanently Installed Appliances
This endorsement is the same as HO 2702 described above
except that permanently installed kitchen or laundry appliances are covered.
HO 2708–Water Back Up
and Sump Discharge or Overflow
Modifies a basic policy to allow coverage for losses to the
dwelling, related private structures or personal property caused by water that
either backs up from a sewer or drain or which overflows from a sump, sump pump
or a similar system. A limit and
deductible must be entered on the endorsement schedule.
HO 2721–Accidental
Discharge or Overflow Coverage - Liquid-Filled Furniture
(Use with all forms except HO 0008)
This property endorsement provides coverage for covered
property that is damaged when liquid-filled furniture accidentally and suddenly
releases the liquid. There is no coverage for the liquid-filled furniture
itself. Also, if the release was gradual or caused by freezing because the
insured had not been responsible in maintaining heat or emptying the furniture,
there is no coverage.
HO 2722–Residence under
Construction - Theft
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
This endorsement broadens coverage under the policy to add
protection for losses due to theft or attempted theft at a covered property
that is undergoing construction. The coverage extends to materials and supplies
that are intended for use in building a residence.
HO 2730–Coverage C - Personal Property - Special
Coverage
(Use with only HO 0004)
This form may be used to convert the HO 0004 policy’s
protection of personal property from a basic to a special coverage form.
However, it further restricts payments on losses involving certain classes of
property such as jewelry, guns, furs, silverware, etc. The form modifies the
basic policy’s Property Coverages and Exclusions Sections.
HO 2736–Extended
Theft Coverage - Described Location Occasionally Rented to Others
(Use with all forms except HO 0008)
This form modifies a homeowner policy’s provisions to
accommodate certain theft losses when part of the residence premises is rented
to another party. Cannot be used on a policy containing either HO 2730 or HO
7029.
HO 2753–Earthquake
Loss Assessment
This form provides coverage for amounts assessed by a
property association to its members for losses caused by earthquake. The loss
has to involve property that is collectively owned. A percentage deductible and
a specified limit apply to each assessment. However, quakes, tremors or shocks
that occur within a 72-hour timeframe are defined as a single occurrence. This
does not pay for an assessment due to the associations’ deductible.
HO 2754–Earthquake
Coverage
(Use with all forms except HO 0008)
This form adds protection against earthquake to coverages
for dwelling, related structures, and personal property. Coverage for this
additional peril does not increase the limits of insurance shown in the
declarations. Further, coverage for earthquake is subject to its own percentage
deductible. There are a number of other limitations including exclusions for
damage caused by earth movement that is not the result of an earthquake (such
as mine subsidence), no coverage for the expense to fill-in land. Damage to
exterior masonry veneer is not covered but can be added if scheduled.
HO 2770–Computer
Coverage
(Use with all forms except HO 0008)
While this form does NOT increase the applicable Coverage C
limit, it does modify the HO policy to add protection for “computer equipment,”
a defined term. Computer equipment includes normal computer hardware and
related software/media. Coverage is provided on a special form basis (sources
of loss are covered unless specifically excluded). Exclusions include loss due
to freezing, leakage, theft (during construction), humidity, wear and tear and
other limitations.
HO 2786–Identity
Fraud Expense
(Use with all forms except HO 0008)
The form adds its own defined terms (expenses and identity
fraud). It provides coverage for certain costs that are directly related to
dealing with being victimized by identity theft. Most of the recoverable costs
involve the time and paperwork necessary to report identity theft to various
parties such as the police, credit agencies, lenders, attorneys, retailers,
etc. The form also provides limited loss of earnings coverage on a daily limit
basis (subject to a modest, aggregate limit).
HO 3048–Related
Private Structures - Described Location
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
This optional endorsement may be added in order to increase
the limit of insurance for any other structure that’s described in the policy.
The amounts of coverage appearing in the endorsement (or shown elsewhere) are
additional insurance for the specified structures and are in addition to the
Coverage B limit.
HO 3061–Scheduled
Personal Property Coverage
(Use with all forms except HO 0008)
Allows an insured to schedule (specifically list) coverage
for ten classes of property that are subject to modest sub-limits under the
basic dwelling policies. Note that this specific coverage REPLACES the coverage
available under a basic policy. The form includes its own limits, property
descriptions, coverage extensions, coverages, exclusions, territory, and
coverage conditions that should be carefully reviewed – especially the loss
settlement terms.
HO 3063–Scheduled
Personal Property Coverage Agreed Value Loss Settlement Terms
(Use with all forms except HO 0008)
This coverage is identical to the Scheduled Personal
Property Coverage provided in HO 3061 except for the Loss Settlement Terms. Agreed value is provided for some, but not all, of the
scheduled property. Review the loss settlement terms carefully.
HO 3068–Scheduled
Glass
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
This endorsement provides glass breakage coverage for plate
glass that is described on the form with exclusions only for Nuclear and War. A
specific limit applies to this property and losses are settled on an actual
cash value basis. Coverage extends to protection for the frames that surround
the glass. The covered perils include breakage and damage from chemicals.
HO 3540–Related
Private Structures Rented to Others
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
This endorsement may be attached to a basic policy that
includes coverage for a related private structure that is being rented or is
available for rental. In order to qualify for coverage, the structure has to be
shown on the form’s schedule (or elsewhere on the policy) and it must be
located on the insured premises. The limit of insurance available under the
policy’s Coverage B does not apply to the rented structures described in this
schedule so the limit shown in the schedule is the most applicable to the
scheduled structure. This endorsement also extends coverage provided by
Incidental Liability Coverage for Business to include these structures.
HO 3542–Office,
Professional, Private School, or Studio Occupancy - Described Location
(Use with all forms except HO 0004)
This optional endorsement can be used to add protection for
a limited number of business activities operated on the insured premises. The
business must be described and it may be located on the residence premises or a
related private structure, which is either on or away from the insured
premises. This extension is for both property and liability sections of the
policy. Slightly revised to conform with
the language of the ’08 edition of the HO program.
HO 3557–Care Provided
for Others
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
To qualify for coverage, the exact activity has to be
described and the number of persons receiving care must be shown. The form
modifies Coverage B—Related Private Structures and Coverage C—Personal Property
in order to accommodate a business activity that is located in a related
private structure and to cover personal property that is directly related to
the permitted business. On the liability side, the form extends liability
coverage by amending the policy wording for Coverage L—Personal Liability and Coverage
M—Medical Payments to Others. An insured has the choice of applying the limit
of insurance shown for Coverage L or selecting a separate limit as an annual
aggregate.
HO 3565–Higher Limits
on Certain Property Used For Business Purposes
(Use with all forms except HO 0008)
This endorsement does not increase the limit of insurance
shown for Coverage C—Personal Property; rather, it increases the sub-limits
provided under the Coverage C Limitations on Certain Property coverage. The
form allows an insured to re-apportion how the applicable policy’s Coverage C
limit applies to property used in a business.
HO 3571–Business
Activities
This extends Personal Liability Coverage to losses arising
from the described business activity shown on the
endorsement. The form includes an option to add protection against corporal punishment.
HO 3801–Watercraft
This extends Personal Liability Coverage to include
protection against liability arising from the ownership, use or operation of
the described watercraft. The form excludes coverage for the following:
·
renting the watercraft to other persons
·
use of the watercraft to carry persons for a fee
·
bodily injury suffered by a person whose
employment is related to the described watercraft
·
loss occurring while either preparing for or
using in a race or performance contest.
HO 3828–Incidental
Coverage for Motorized Vehicles (Low Power Vehicles)
(Use with all forms except HO 0008)
This form changes a homeowner policy so that it provides
coverage for limited liability exposure to losses related to certain types of
motorized vehicles. The types of vehicles that qualify for coverage are those
that are not subject to motor vehicle registration. Further, coverage does not
apply to motorized bikes, mopeds, golf carts or any vehicle that moves faster
than 15 miles an hour.
Parties who have a vicarious legal responsibility for harm
caused by such vehicles qualify for coverage, but only with respect to their
level of liability for a given loss.
HO 3831–Scheduled
Motorized Golf Cart Coverage
(Use with all forms except HO 0008)
The policy is amended to provide coverage for golf carts
that are described on the form’s schedule or on the policy declarations. Golf
carts are covered against damage caused by any of the sources of loss found
under Coverage C—Personal Property, as well as for collision. However, NO
coverage exists for golf carts that carry persons or property for pay; nor is
there coverage for carts that are used in or preparing for a race or similar
contest.
HO 3832–Golf Cart
Liability Coverage
This form changes a homeowner policy so that it limits
Incidental Liability coverage to only those golf carts specifically described
in the endorsement. Therefore, the coverage is more restrictive than the basic
form.
HO 3864–Snowmobile - Liability
Coverage Only
This extends Personal Liability Coverage to include incidental
liability protection arising from the ownership, use or operation of the
described snowmobile – when used off public roads.
HO 4001–Personal
Injury
This allows an insured to add coverage to a base policy for
his or her liability that’s related to personal injury losses. The form
includes a definition of personal injury that includes false arrest or
imprisonment as well as wrongful eviction, libel, slander, and other causes of
loss. However, no coverage is provided for such losses if they stem from
allegations of sexual molestation or from a communicable disease. A number of
other sources of loss are also excluded.
HO 4009–Waterbed
Liability
(Use with only HO 0004)
This form modifies Coverage L–Personal Liability to provide protection against property damage
caused by a waterbed. However, there is NO coverage for damage to the bed or
any of its parts.
HO 4505–Windstorm or
Hail Percentage Deductible
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0008)
Attaching this form to a homeowner policy modifies how
deductibles apply for windstorm or hail losses. Instead, either the much larger
flat deductible or the percentage deductible entered on the schedule applies.
The revised deductible applies to all coverages with the exception of a number
of incidental coverages.
HO 4815–Actual Cash
Value
(Use with HO 0001, HO 0002, HO 0003)
This form changes a base policy’s loss settlement provision
from a replacement cost to an actual cash value basis for Coverage A, Dwelling
and Coverage B, Related Structures.
HO 4832–Replacement
Cost Loss Settlement Terms - Certain Non-Building Structures on The Described
Location
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
This is an option that offers broader coverage for property
that is typically considered as outdoor fixtures that, under a base policy, usually
are settled on an actual cash value basis. This form allows an insured to
arrange for added protection on, in specified instances, a replacement cost
basis. Generally, the coverage applies to the type of expensive modifications
made when homeowners invest in upgraded non-wood patios, walks, fences, walls,
and driveways
HO 4835–Roof
Surfacing Amendment - Actual Cash Value Terms For Windstorm or Hail Loss
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
This is an option that subjects roof surfaces (and other, selected types of property) to loss settlement on an
actual cash value basis when they are damaged or destroyed by storm winds or
hail.
Note: A
definition of roof surfacing is introduced that is more detailed in explaining
all of the various roof items that are subject to this limitation.
HO 4836–Roof
Surfacing Amendment - Actual Cash Value Terms
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
This is an option that subjects roof surfaces (and other,
related types of property) to loss settlement on an actual cash value basis
when they are damaged or destroyed by eligible sources of loss.
Note: A
definition of roof surfacing is introduced that is more detailed in explaining
all of the various roof items that are subject to this limitation.
HO 4843–Additional
Limits - Coverage A, B, C, and D
This endorsement increases the Coverage A limit to the
current replacement cost of the insured residence at the time of loss if the
named insured meets all conditions of the endorsement. Coverage B, C and D
limits are increased by the same percentage used to increase the Coverage A
limit.
HO 4844–Specified
Additional Amount of Insurance - Coverage A
This endorsement increases the Coverage A limit of insurance
by the percentage stated in the schedule but only if the amount of loss exceeds
the stated Coverage A limit.
HO 4851–Increased
Coverage A Limit - No Private Structures
(Use with HO 0001, HO 0002, HO 0003, HO 0005)
Attaching this form to a base policy allows an insured to
add the insurance limit shown under Coverage B—Related Private Structures to
the Coverage A—Residence insurance limit. However, this can be done only if the
total replacement cost of any related private structures on the insured
premises is less than $1,000 (does not include driveways, fences, sidewalks,
and in-ground pools).
HO 4855–Replacement
Cost Loss Settlement Terms - Personal Property
(Use with HO 0001, HO 0002, HO 0003, HO 0004, HO 0006)
Purchase of this form changes the loss settlement on
personal property, appliances, window air conditioning units, carpets, awnings,
jewelry, furs, photographic equipment, and a number of other items, from actual
cash value to a replacement cost basis. Replacement cost coverage is not
available for pieces of art, antiquity, or rarity, which may not be duplicated.
HO 4856–Modified
Replacement Cost Terms
The loss settlement provision is amended to replace the
standard 80% coinsurance requirement for replacement coverage with whatever
percentage is entered in the schedule. All other conditions remain the same.
HO 4857–Functional
Replacement Cost Loss Settlement Terms
The loss settlement provision is amended to allow a cheaper
alternative for repairing or replacing damaged or destroyed property. Use of
this form allows the insured to purchase a lower amount of coverage and allows
the insurance company to compensate an insured by making repairs with materials
that are not the quality of what has been lost but instead is a current equivalence. The insurer may also make settlement
according to actual cash value. An example could be replacing plaster walls and
ceilings with dry wall.
HO 6016–Additional
Insured - Landlord
The definition of insured under the Personal Liability
Coverage is modified to include the landlord listed on the endorsement but only
with respect to the property leased from the landlord and only while the
insured is living at the premises.
Note: The
endorsement states that the person or organization named in the endorsement
WILL be notified if the policy is canceled or non renewed.
HO 6033–Other
Residents of Your Household
(Use with all forms except HO 0008)
The definition of insured is modified to include the person
listed on the endorsement. This impacts
Coverages C., L. and M sections of the policy which are amended in recognition
of this change. The insured must notify the insurance company within 30 days of
the named person no longer being subject to this endorsement.
Note: The broader definition only applies to
liability arising from the actions of the listed individual while he or she is
a considered to be a resident member of the named insured’s household.
HO 6034–Additional
Interests - Described Location
This form permits adding up to three parties who (along with
any mortgage interest appearing on the declarations) also have an insurable
interest in the covered property.
Note: The
endorsement states that the person or organization named in the endorsement
WILL be notified if the policy is canceled or nonrenewed.
HO 6036–Additional
Insured - Student Living Away from the Described Location
(Use with all forms except HO 0008)
This is option removes the requirement that a relative
student must be under the age of 25 in order to be an insured under this policy
while attending school and living away from the named insured’s primary
location. The name and address of the student plus the name of the school must
be shown.
HO 6041–Additional
Insured - Described Location
(Use with all forms except HO 0004)
This form allows coverage to be extended for additional
persons or organizations that have a valid interest in the property protected
by the policy. The additional insured’s insurable interest is protected under
Coverages A–Residence, B–Related Private Structures, L–Personal Liability and M–Medical Payments.
Note: The
endorsement states that the person or organization named in the endorsement
WILL be notified if the policy is canceled or nonrenewed.
HO 6072–Additional
Insured - Coverages A, B, C, L and M
(Use with all forms except HO 0004)
This form allows coverage to be extended for additional
persons or organizations that have a valid interest in the property protected

structures that are at other locations. The structures cannot be used as a
residence or for business purposes. The insurance limit shown on the
declarations for Coverage B—Related Private Structures applies and settlement
is based on actual cash value.
HO 6218–Scheduled
Related Private Structures - Away From the Described Location
(Use with HO 0001, HO 0002, HO 0003, HO 0005, HO 0006)
This form allows an insured to list additional locations for
protection under the policy’s related private structures coverage. The
structures cannot be used as a residence or for business purposes. A separate
insurance limit is shown for each scheduled private structures.
HO 6220–Association
Deductible Coverage - Additional Locations
(Use with all forms except HO 0008)
This endorsement allows the insured to specifically schedule
a limit for Incidental Property Association Deductible Coverage rather than use
the automatic limit of $1,500. This endorsement does not extend to cover an
earthquake or volcanic eruption deductible.

HO 6243–Office,

This form provides limited property coverage for permanent
improvements that an insured, as a tenant, has made at the specified resident
location.
HO 6266–Coverage C - Higher

(Use with all forms except HO 0008)
This option allows an insured to schedule (list) one or more
additional locations and specify higher, separate limits of coverage for
personal property contained in each residence.
HO 6267–Other
Residence Coverage
This optional form allows an insured to add an additional
residence for coverage under a single policy. The form includes information for
complete rating of the structure and limits may be shown for Coverages A, B, C
and D.
HO 6270–Additional
Residence Premises Rented to Others - Liability Coverage Only
An insured may use this form to add liability coverage for
up to five additional residences that an insured rents to other persons. The
form extends protection under both Coverage L—Personal Liability and Coverage
M—Medical Payments to Others. The location and number of families must be
included on the form (or on the policy declarations).
HO 6516–Protective
Devices
This endorsement acknowledges that the policy’s premium
reflects a credit granted for an alarm system, sprinkler system or similar
protective device. It also obligates the named insured
to maintain the system and to tell the insurer if the system is removed or if
it no longer works. This is a warranty and coverage will be voided if the
conditions of the endorsement are not met.
HO 6544–Change
Endorsement
This form is used merely to instruct the insurer to make
coverage changes, including premium and payment changes.
HO 6576–Rating
Information
This endorsement is a separate form that allows the complete
rating of a personal residence on base coverage Forms 0001, 0002, 0003, 0004, 0005,
0006, or 0008. It contains all of the rating information that is typically
included on a homeowner or personal liability insurance application. The form
requires that the applicant identify any special circumstances such as the
existence of any business activity, whether the submission includes any
domestic employees or business activities, whether there is any watercraft
liability and similar items.
HO 6578–Insurance by
More Than One Company
This endorsement acknowledges that more than one company
provides coverage to the insured’s property. The form indicates the total
existing coverage for Coverages A through D and obligates the insurer to
provide a specified percentage of the total shown in the form (or elsewhere).
There is also space on the form to identify the policy that provides the
liability coverage to the insured.
HO 6581–Premium
Payments
This is merely a notification to the insured that the
insurance premium is:

payable annually,
·
based upon the rates which are current at the
time of the policy’s anniversary, and
·
subject to the current forms and endorsements.
HO 6820–Private Fire
Protection Condition
This form affirms that, for a named insured whose covered
residence is served by a private (rather than public) source of fire-fighting
protection, the named insured agrees to maintain that service throughout the
life of the HO coverage. This is a warranty and coverage will be voided if the
conditions of the endorsement are not met.
HO 7029–Unit-Owners
Coverage C - Special Coverage
(Use with only HO 0006)
This form may be used to convert the personal property
coverage from a basic to a special coverage form. There are restrictions on
payments for losses involving certain classes of property such as jewelry,
guns, furs, silverware, etc. The form modifies the basic policy’s Property
Coverages and Exclusions Sections.
HO 7032–Unit-Owners
Coverage A - Special Coverage
(Use with only HO 0006)
This form may be used to convert Coverage A from basic to a
special coverage form. It replaces the basic policy’s Perils Insured Against
Section. However, the basic policy’s section, Exclusions That Apply to Property
Coverages, still applies.
HO 7033–Unit-Owner Rental to Others
(Use with only HO 0006)
This endorsement may be attached to a basic policy that is
covering a condominium-unit owner who is renting or is making his or her unit
available for rental. The form modifies Coverage C—Personal Property, Perils
Insured Against extends the Incidental Liability Coverages section.
HO 7500–Farm
Liability Coverage
This form is designed to extend protection under a basic
policy to cover liability arising out of a farming operation. Specifically, the
endorsement replaces the basic policy definitions of business and domestic
employee, modifies the definition of insured and adds the definitions of farm
employee, and farming. The new and revised definitions apply ONLY to losses
involving the farm coverage. The form adds an incidental liability coverage
provision for custom farm work. The
form also adds several coverage limitations so that the endorsement is suited
for covering modest farming exposures.
HO 7515–Custom Farm
Work
This extends the farm personal or commercial liability
coverage to provide protection against damages resulting from an insured’s
described custom farm work.
HO 7520–Employers’
Liability and Medical Payments Coverage - Farm Employees
Amends the Personal Liability Coverage by allowing an
insured to identify his employee exposure and to elect coverage for injury to
farm employees as well as liability coverage for employees who cause injury or
damage while performing their jobs. Rating basis may be according to the number
of full and part-time employees (man-day) basis or upon a wage basis. The
endorsement’s man-day approach is preferred as it best reflects the employee
related liability exposure.
Note: This
coverage is not to be confused with employment practices coverage as this form
excludes any acts involving hiring, firing, demotions and other related acts or
policies.
HO 7525–Farm
Chemicals Limited Liability Coverage
This amends Coverage L–Personal
Liability and Coverage M–Medical
Payments to Others to include partial protection against liability resulting
from an insured’s use of farm chemicals on the insured premises. The insurance
limit selected on the form (or shown on the declarations) applies to an annual
aggregate basis. The chemical use must be in connection with routine farm
operations. Coverage is excluded for damages caused by chemicals released via
aircraft, indirect losses, illegal farm operations and actions involving
pollution monitoring, testing, assessment, or remediation.
HO 7530–Fruit or
Vegetable Picking By Public
This amends the Personal Liability coverage to protect
against losses arising out of the named business activities. The fruit or
vegetable picking liability has an option to include coverage for ladder and
off-ground picking.
HO 7540–Incidental
Farming
Amends Coverages L–Personal
Liability and M–Medical Payments to
Others to cover the liability related to the minor farming activity either on
the insured premises or by an insured at a described location.
HO 7545–Livestock
Collision
(Use with all forms except HO 0008)
The property coverage may be written to cover loss (death)
of animals that occurs because of a collision with a vehicle on a public road.
The coverage applies only when any insured or his or her employee does not
operate the vehicle. Coverage applies to cattle, horses, mules, donkeys, hog, sheep,
or goats. $400 is the absolute maximum that may be paid for any individual
animal and is subject to the market value and the named insured’s interest in
the animal.
HO 7550–Products Annual Aggregate Sub-Limit
Coverage is modified under the policy’s Personal Liability’s
insurance limits to act as annual aggregate limits for any covered losses
involving damages caused by products which an insured makes, sells, handles, or
distributes.
Note: The conversion to an annual aggregate applies
to Coverage L–Personal Liability.
Coverage M–Medical Payments still
operates on a per occurrence basis.
HO 7555–Horse
Boarding Operations
Personal liability coverage is extended to include
horse-boarding operations that are described in the endorsement at the location
indicated. This is location specific coverage. There is no coverage for injury
or damage caused by animals that escape.
